Root Cause Failure Analysis
Root cause analysis of failures in equipment allows solutions to be developed that mitigate future issues.
Using advanced simulation techniques in combination with observed failure modes and measurement allows root cause failures of components to be replicated and proposed design modifications to be tested in a virtual environment.
Determining Root Cause
Establishing the root causes of failures requires the creation of representative simulation models that can replicate the observed behaviour and failure modes given the expected (or measured) loads and boundary conditions. We follow a systematic process to replicate root cause failures by iteratively adjusting possible loading scenarios and boundary conditions to match the observed behaviour, with subsequent modification and testing of designs performed in consultation with clients to avoid future failures.
Our approach to determining root cause failures includes:
- Generation of baseline simulation model based on CAD models/drawings and site measurements.
- Iterative engineering analysis and adjustment of loads and boundary conditions to replicate observed behaviour and failure modes.
- Modification of simulation model to incorporate proposed design modifications (with client consultation) and further testing/validation to confirm avoidance of failure under same loads and boundary conditions.
